Request For Use of the DEC Facility

Instructions for Completing Building Use Form

1. DOWNLOAD the Building Use Form (in MSWord format) by clicking HERE

2. SAVE the form to your computer

3. FILL IN the form by using your "Tab" button to move between fields

4. RE-SAVE the form to maintain your input information

5. SEND the form, as an attachment, to the Building Use Coordinator at:

6. KEEP your copy of the form for your records

The Building Use Coordinator will contact you regarding your submission as soon as possible.  Please be sure to fill out as early and with as much information as possible to help us avoid delays with your building use request.

This form is in Adobe PDF format.  If you are requesting use of the DEC Facility via this "hard copy" form, please print and return the completed form to the DEC office.
